By completing the form below, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only. Conga’s Privacy Policies and your rights as a data subject (if applicable) are available at https://conga.com/privacy/.
This agreement represents my entire understanding with Conga.
I have read this Agreement prior to accepting it, and I understand its contents. This agreement may not be amended unless Conga and I agree in writing.
Subject to approval (as defined below), I grant Apttus Corporation, its subsidiaries and affiliates (“Conga”) the right to incorporate any trademarks, logos, name, likeness, images, voice, and performance on film, video, audio, computer files or otherwise (“Company Assets”) for use, in whole or in part, in perpetuity worldwide by Conga and those it designates for promotional, educational, or other purposes, in any format or media now known or later created (“Promotional Materials”). I grant and assign to Conga all right, title and interest in and to the Company Assets, including, without limitation, copyright. I confirm that: (1) I have the right to enter into this agreement, (2) my performance and the rights granted in this agreement will not conflict with or violate any commitment I have with any other person or entity, and (3) Conga has no financial obligation to me for the Promotional Materials or this agreement. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse settings within an unrestricted geographic area. Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to:
- Presentations
- Courses;
- Online/Internet Videos;
- Media (social media, emails, website); and
- News (Press) Release
I will have the right to approve in writing, the final version of the Promotional Materials that include Company Assets, provided that such approval will not be unreasonably withheld and provide that Company will approve or disapprove of the Promotional Materials within three (3) days of receipt. I represent that Conga will have no further obligation to obtain any further approval from me to edit or modify the approved Promotional Materials, so long as such modifications are within the scope of the licensed rights as set forth above. I release, discharge, and agree to hold harmless Conga, and all persons or entities acting under its permission or authority, from any liability (including, without limitation, negligence) for any injury that may occur to me while performing or appearing in the Recordings. I also hereby release and discharge Conga and its agents, assigns, licensees, and sub-licensees from any and all liability to me or to my successors, heirs or assigns for any claims, including, without limitation, claims for libel, defamation, copyright infringement, invasion of privacy or right of publicity, arising out of the exercise of rights granted under this agreement.
